PSYCHIATRIC MEDICATION MANAGEMENT
Medication Therapy Management (MTM) designed to help individuals navigate their mental health journeys with greater precision, safety, and efficacy.
WHAT IS MEDICATION MANAGEMENT?
Medication management is a proven therapeutic strategy for mood disorders and chronic illnesses. It involves precise monitoring and administration of medications by healthcare professionals, ensuring individuals use them safely and effectively. The medication management process begins with a comprehensive evaluation of the individual's health history, current medications, and specific needs related to their condition. Based on this thorough assessment, a tailored treatment plan is developed, clearly outlining the type, dosage, and timing of medications. Progress is then tracked, making necessary adjustments to the medication regimen, to optimize treatment outcomes and effectively manage any side effects.

How Does Medication Management Work?
1. Diagnosis
First, we’ll evaluate your health status. We’ll use talk therapy and other assessment strategies to diagnose, examining both root causes and symptoms. For instance, a client with sleep disturbances might have PTSD, which contributes to those effects.

2. Treatment Plan
Once a diagnosis has been established, we will come up with a treatment plan to address symptoms and help the you enjoy a better quality of life. Up-to-date, refined strategies will be used to ensure you receive the right treatment for what you are going through.

3. Prescribe Medication
Psychiatric medication management services prescribe the right drug for your needs. Choose a therapist to support you through this journey. Our licensed prescriber will write a script for you to fill at a local pharmacy.

4. Monitoring & Evaluating Response
Our nurse practitioner will monitor and evaluate your response to the prescription which ensures you receive desired effects without negative side effects that hinder your quality of life. Routine follow-ups and check-ins help maximize the benefits of the medication.
